Patient Registration

To register with the practice

writingIf you live in London NW11 or in parts of the surrounding Post Code areas you are welcome to join the practice. Reception can confirm whether you are in the practice area. If you wish to register as a private rather than NHS patient speak to reception.

All Patients

The NHS provides services to UK residents only and we are required to verify your residency status as part of the registration process. Please enclose a document confirming your current address and status (a utility bill in your name, bank statement, tenancy agreement or a letter from your university or employer) together with a copy of your passport with the registration forms. (We have to check this later as part of the registration process).

You will need to complete the following forms:

a GMS1 form,

a Summary Care Record Form (opt-in or opt-out) and a

practice health questionaire (there are separate forms for adult and children under age 16)

which can be found at the foot of this page. Please return them to the surgery. They cannot be faxed or emailed as the NHS requires us to have an original signature on the GMS1 form. If you have a medical card (FP4) please let us have that at the same time. If you have lost your NHS card complete form GMS1 but include your NHS number (you may need to contact your previous GP to obtain this).

Complete the practice questionnaire ensuring all parts are completed in full as incomplete forms will delay registration. Pay close attention to the questions about your country of origin and the date of entry to the UK.


IMPORTANT: The information you give us will be stored on a computer database held by the practice. Under new arrangements introduced by the NHS some of this information will be uploaded to the `Summary Care Record’ which makes this data available to other NHS users. The NHS (not us) assumes your consent to your information being shared in this way unless you state otherwise. If you do not want your information uploaded to the `SCR’ complete and sign the declaration on the SCR form. You can change your decision about this at any time.


The NHS system can be confusing and this link may be useful for Non-English speakers and others unfamiliar with the NHS.

(If you are unable to provide the information required to register as an NHS patient we can still see you as a private patient but then you are responsible for any charges incurred).

We shall write to you after we have received and processed your forms inviting you to a `registration appointment’. This usually takes 10 working days (although if often quicker) allowing for any postal delays

All patients registered with The Hodford Road Surgery are registered with Dr Cavendish although they may book with any of the other doctors who work at the practice.
